    Am I right in guessing that 99% of these are due to cable theft?

    Not really. Could be a faulty module or they can be busy with a upgrade to the exchange or there could be a power outage.

    Upgrade to 8Mb/s , I see no mention of the xpcm script being run for the upgrade - so its maybe a hardware upgrade
