I work as a developer for call centre software (among other things) in an insurance company (one of the largest in the country). Our call centre is over 1000 agents and we just use a server with CentOS and Asterisk (yep 1 server for over 100 concurrent calls).
We never have problems with the Asterisk server going down, we do however experience problems with outside lines (no matter who you go with, it happens).
Personally we've tried all these small providers and their quality, expertise (big problem) and reliability all suck. We've gone with one of the large Cell phone companies now. Fail over is with Telkom PRI x4 which is the most reliable (but more costly).
Personally I doubt you could beat Telkom BRI and Asterisk for a small business top quality PBX solution.

iceman23: Switch Telecom's call rates are decreasing significantly on 1 May 2011. Hold out for a few more days and you should receive the notification of the new rates via email.

Superspeed. The exchange is across the road, the noise and attenuation are at very good levels. I will post those tomorrow.
Also, we dont have a current onsite pbx so internal calls are routed out and then back in. That is one of the reasons we want to move in house.

Access Lists on voice breakouts. These would only allow sip and rtp traffic between the specific customer pabx and service provider Session Border Controller. On the SBC you would have Access Lists there as well to only allow traffic from a specific IP.

I dont recommend anyone to have the InHouse VOIP because now all business telephone system providers are reliable and they are really cheap. Quality of the voice is also better. I dont understand the reason why do people want to have it InHouse.
Some people choose inhouse PBXs due to size of the installation or custom requirements and interaction with other applications or databases. You are also putting all your faith in your internet connectivity being up 24/7 with a hosted solution so you either need redundant connectivity or you host it inhouse and have a mix of VoIP and fixed line services to tie you over while your link is down.

Hosted PBX is just another word for proprietary, the cost of the solution is simply moved away from the actual PABX to the end user devices and in most cases these need to be purchased, yes these too need to be purchased when deploying a in-house solution but the fee per extension falls away and you don't have pay for every single feature you want.
