I would like to ask if it is possible for a client to use one sip on a Yealink T46G for their receptionist to direct calls to cell phone numbers instead of internal extensions.

Yes. We use a 3CX system that allows transferring calls to external lines (Eg cellphone number) and also has a mobile app so that calls to the app are internal transfers
