I'm with you on the surname thing -- I get it a lot. Really don't know why people do it.
While I would sometimes greet someone as Mister <Surname> (as a joke), constantly using someone's surname just feels silly.

Only one person gets away with calling me 'sweetie' and that is because she is the mother of my children and I love her to bits.
Anyone else use that and they will not find the body.

I have this habit of calling people 'dude' or female friends 'darling'. But only if they know its not condescending or nasty, its just a term of familiarity for me. (And I'm horrible with names.)
