Affidavit for SIM swap? You understood it wrong. Affidavit is requested in support of your claim that your SIM have been stolen. It is legal requirement. You wrote about stolen SIM in another thread.

Nope, affidavit for my lost id. Seems a temp ID would not cut it.
I went to the other cell c shop @ the mall and they helped me just fine
