They should just redo the entire fibre install. Not even Octotel's network can be that bad.
Do you have neighbours with fibre too? Stable for them?
Have you eliminated every single point of failure from your 'house' to the CPE? Different routers, etc. Checked settings like 'dial on demand' ?

Without seeing where the source of the traffic is coming from there isn't much we can do as an ISP. Try with our DNS and see if there is any variance? MFN/Afrihost (300Mbps) using CF DNS I get 295Mbps
OS/CISP (500Mbps) using CF DNS I get around 100Mbps
