I have been experiencing exactly the same problem for three days. Tried every browser possible but still an empty space where the form should be.

I tried to get the "confirmation letter" without success.Worked for me after hours of trying to find the confirmation. Thank you
Went to FNB( CT) on my son's appointment date and they didn't even asked for the letter.
They just wanted his Birth Cert. and one parents ID.
Both parents had to sign thou.

AFAIK only FNB Portside in Greenpoint do passports.
FNB extends its smart ID, passport collections to Cape Town branch
FNB customers, through a partnership with the Department of Home Affairs, can now pick up their new documents at its Portside branch in central Cape Town.

Trying to register and you can immediately tell that very little effort has gone into this.
I'd love to know how much this embarrassment cost.

Went yesterday too collect an id that I got an sms for, and the passport had been sitting there since the 3rd.
They didn't notify me....
Anyways glad I've got it

I suggest you get quotes from these companies, then go with the best one. They are pricey.
I haven't used them but I have been recommended them by others as well.
I heard Unabridge BC can take a while though (up to 1 year depending on how complicated the person is)
There is no easy way with BC unfortunately.
