I think I'm going senile...
I have a Galaxy s2, rooted running ICS. I have been using "connect as USB storage device" function in order for my laptop to see the Galaxy as a USB storage device in order to backup my Quickbooks.
Don't know what I did but I no longer have that option available to me. Only have connect as Media Device or Camera. (USB debugging selected or not)

I'm sure I have see similar problem but, for the life of me, I can't find it.

Any ideas as to what's up with the phone (or me)?

Have you changed kernel's recently? That could be the case.

However you can still connect as a USB device, you just have to do it manually now.

If memory serves:

Settings -> More... -> USB settings -> Turn on USB storage.
