How to write an affidavit (for someone else to collect my drivers license)

Hello

I need a family member to collect my renewed drivers license (since I live abroad). I understand from here (https://www.westerncape.gov.za/service/driving-licences) that I need to write an affidavit authorising my family member to collect my license.

Does anyone know exactly how to write this letter? Is there some required format and wording? I assume I can then get this stamped at any police station.

I "A" give permission for"B" to collect "C" on my behalf. A and B ID numbers included with copy of ID!

Acasia in Pretoria didn't accept an affidavit signed by a commissioner of oaths when I tried to collect my daughters drivers licence. She already had left the country when I tried to collect her licence so she could not sign a new one at the police station.

Lots of places with idiots not knowing what they doing or what is acceptable within law.

Commissioner of oaths is accepted in law to take a sworn oath from a deponent which is what an affidavit is.

Police have better things to do than waste their time signing certified copies and taking sworn statements for private use. There's no difference in making the oath in front of a police officer or a commissioner of oaths except their stamps.

I'm a commissioner of oaths.

You are quite right, sometime ago the police even requested it in the media that all commissioners of oath should help them to relief their workload.
But it didn't help in my case.
