Right, this will take a moment... First the prep work...

Never sure if it made a difference but I've always used a .com mail address for my US Store Account. If you have one, great, if not, GMail is your friend. Don't use a GMail address if you're already using it for your SA store account 'tho. Your mail address gets used for e-mail verification while you're opening the account.
Second you need to identify an actual US address... A hotel or restaurant will do... You need the street address as well as the telephone number. I'm betting more people live at 1 Infinite Loop (Apple's actual address) than most places in the States. I picked an address in New Hampshire 'cos it's one of the five states in the US without sales taxes.

Right, next you open iTunes and log out of your SA account.

Next, click on the App Store link in iTunes and scroll down to the bottom of the page and click on the US flag icon.

Find any free app and click on the "Free" button to purchase it... You'll get asked to create a new account in the pop up that appears.

Enter the .com mail address and your password (your password can be the same as your SA store account password)

Answer all the questions 'till you get to Payment Method... Select "None".

Next is address and phone number and then you'll be sent the mail verification but you're pretty much good to go with the US account right there.

NOTE: iTunes store vouchers now become your best friend. There are a couple of threads on MyBB discussing this and where to get 'em... You'll have the US store account but your local credit card wont work in it. You need to buy US store vouchers and redeem them in the US store to buy books or movies and so on... I've been using a site called buyfrompowerseller.com but there are others out there.\

Anything else, just ask here

I put yahoo.co.uk (not com), followed everything else somebody posted before and it worked, no problem.
