Here is an idea for the banks you can pay me for when you use it. After I have logged into my account, give me a row of 20 visuals, icons or pictures. I have 3 chances to click on the correct one I have pre-selected earlier on in a different process like when I opened my account etc. etc.
I have to click on the right visual to continue otherwise my account gets locked.
This way I get password, user name, SMS and visual confirmation for my own security.
Even if the criminal can get your login details they would be hard pressed to know which visual you have pre-selected as an additional security layer.
The visuals of course gets randomized every time I use my account.
