It will be the only form of television broadcast by 2015. This is excluding rural areas.

Its 2015 at the latest as far as I know for the entire country.
I dont think it is "law", but internationally, after 2015 it will no longer be a protected frequency (The current analogue signal)

There will definitely be some analogue signals around after 2015. Areas of the Northern Cape for instance...
