At the start of the video there seems to be a slow moving white vehicle (taxi or bakkie) that is joining traffic from the roadside, and the truck may have went left to avoid it. There are two lanes going left into Flanders drive and the vehicle is attempting to merge onto the M4 cutting across these lanes.

Breaks do fail, seen it myself comming down the hill into maritzburg

100%. Seen it on that hill a few times, and obviously we've seen the results at the bottom of Fields Hill as well...

Without any extra info it would not surprise me if that driver fell asleep behind the long haul truck steering wheel.

If you lose control of any vehicle while actually awake you would attempt to break but this truck just steered right through the fence without a hint of breaking from viewing the video alone.

I saw one happening right in front of me, it's scary **** but the driver got it into the arrestor bed and it was all good. Besides that I probably saw 10+ trucks in the arrestor beds after years on the road. I have a lot of respect for those truck drivers.
