The US maintain this through their SSBN's.
As long as each side can ensure they will have enough nukes to strike back with, they can apply to the MAD doctrine.
The reason the Russians wanted to base missiles in Cuba, was to even out the numbers with the USA. In 1963 the Americans had far more nukes than the Russians. Plus a bigger advantage in better technology.
In today's sphere, the need to have bases so close is negated by SSBN technology.
