Name and Shame: Unauthorised Debit Orders Thread

I got the same debit order deducted this morning 2 Nov 2018 of my account. Refer screenshot attached. Reported to my bank. They said have to complete a debit order reversal form and stop payment form and send back. And they reporting it to fraud team for investigation.
