Eskom managers overruling engineers caused catastrophic Kusile chimney collapse

Eskom management's instruction to increase the load of Kusile Power Station Unit 1 to full capacity, against the advice of its engineers, caused the disastrous collapse of a duct which has contributed roughly two stages of load-shedding since late October 2022.

That is according to an internal report by the utility's assurance and forensics department, which was seen by the Sunday Times.

Ahhh, the standard "We can't hold anyone accountable because it was a verbal instruction".

Its simple then, if a "manager" issued a verbal instruction and it was implemented, the person who implemented the verbal instruction which has led to this issue should be fired.
 
Sounds like Chernobyl, where after watching the series I put the whole root cause down to communism. Everyone schit scared to stand up to their boss, who in turn who are all schit scared of standing up to their bosses.

Good analogy for where we are now as a country, i.e. being fscked over by idiots in power.
 
Yes, a verbal instruction with no audit trail is a time-honoured way of shifting responsibility. Plenty of "managers" at Eskom, guess the only ones who are in the clear were those gallavanting around the world talking about green power rather than focusing on coal plants?

They followed in the footsteps of NASA also with regards to the Challenger!



What a wash-and-repeat worldwide phenomenon ...
