With conventional weapons. They have never used weapons such as biological or chemical weapons, things that would turn world perception against them. Using Nukes would destroy their standing on the world stage and I doubt that Russia or China would defend them if they did.

Yes, I imagine the US kidnapping of Maduro and the Assination of Ali Khamenei has made them a bit paranoid. South Korea most likely, Seoul is only 60km from the North Korean border. The threat of them nuking Seoul in retailiation for a US assasination would probably give the US pause.
