Amazon-owned Boeing 767 cargo plane crashes after overshooting runway in Miami

Edit: Pretty damning pilot errors, with conclusive data and video footage. Unstable approach in bad weather, too high and too fast, landed halfway along the runway, crosswind lifted the left landing gear wheels which prevented the thrust reversers from deploying. Pilots should have gone around rather than trying to force a landing. Condolences to the casualties on the ground.
Crosswind keeping left gear off the ground (if true) points to poor technique. That's why you're taught to keep ailerons into the wind even AFTER you have touched down.
 
Had a look at the csv data for this flight. It hardly slowed down on the runway.

I forgot to add: Flight spoilers did not deploy, so it 'floated' all the way down the runway. Auto braking cannot activate unless there is weight on (both sets of) wheels. Total cluster-F...

5 Dead After Amazon Prime Air-Branded Boeing 767-300F Overruns Runway In Miami - Simple Flying​

Five people are dead and others are injured after an Amazon Prime Air-branded Boeing 767-300F went off a runway at Miami International Airport on Sunday, September 6th. The excursion happened shortly after the aircraft touched down.

 
The first NTSB press conference was less than useless IMO, except to confirm certain basic facts:
The plane hit two vehicles, including a white van carrying seven people within the airport perimeter, and a Toyota with three passengers.
Investigators will examine the flight history, crew experience and training, radar data, and aircraft performance. Separate teams will look at the plane's systems and engines, while a meteorologist will assess the weather conditions at the time of landing.

EMAS isn't going to do anything for an aircraft travelling at 112kts:
The NTSB will also examine whether the runway should have had an engineered materials arresting system (EMAS), which is designed to slow and stop aircraft that overrun a runway. Homendy said this would be a "key" part of the investigation.

Cleanup and repairs will obviously take a while:
Two of the four runways at the airport remained closed following the crash. Passengers have been warned to expect further disruption after Sunday's cancellations affected the arrival of aircraft and flight crews.
 
Historical note: The 'weight on wheels' sensor and software logic were added to the B767 following the 1991 Lauda Air disaster. Uncommanded deployment of a thrust reverser led to an in-flight breakup. Pilot manuals clearly state the conditions under which the various systems operate, and the requirement to firmly plant the landing gear:
On 26 May 1991, the Boeing 767 operating the route crashed following an uncommanded deployment of the thrust reverser on the No. 1 engine during the climb phase, causing the aircraft to enter an aerodynamic stall, uncontrolled dive, and in-flight breakup, killing all 213 passengers and ten crew members on board. It is the deadliest aviation accident involving the Boeing 767, and the deadliest aviation accident in Thailand's history. The accident marked the 767's first fatal incident and third hull loss.
The incident prompted Boeing to modify the thrust-reverser system to prevent similar occurrences by adding sync locks, which prevent the thrust reversers from deploying when the main landing-gear truck-tilt angle is not at the ground position.

If the speed brakes never deployed then that's probably another thing they missed. Literally the first thing the pilot-monitoring does after touchdown is to verify and call whether the speed brakes are up or not. If not, then they should pull the speed brake lever manually.

"DEVASTATING" says NTSB Chair: Initial Update Amazon 767 Crash That Killed 5 - Taking Off​

FIVE PEOPLE ARE DEAD after an Amazon Air cargo jet overran the runway at Miami International Airport on September 6, 2026.

This update walks through the NTSB's first press briefing with Chair Jennifer Homendy — what investigators confirmed, what they flatly refused to say, and one published number about that runway that almost nobody is reporting.

21 Air Flight 7598, a Boeing 767-300 freighter operating for Amazon Air, arrived from San Juan, Puerto Rico and overran Runway 30 on landing.

According to Chair Homendy, the aircraft struck navigational aids and equipment, then hit a van carrying seven airport contract workers INSIDE the airport perimeter, went through the perimeter fence, struck a Toyota Corolla Cross traveling on Northwest 67th Avenue, and came to rest roughly 1,300 feet past the end of the paved surface. It caught fire.

Five people were killed and five injured. Those figures are attributed to the Miami-Dade Sheriff's Office and the county mayor — the NTSB has not issued its own casualty count. No victims have been publicly identified.

Econoline van struck by plane belongs to Professional Ocean Service - WPLG Local 10​

NTSB Chairwoman Jennifer Homendy said the Boeing 767-300 on Prime Air Flight 798 for Amazon struck a white 2012 Ford E-150 Econoline van inside MIA and a Toyota Corolla Cross LE that was on Northwest 67th Avenue outside of MIA.

You know as well as anyone that he was referring to the US, not these specific two pilots. The US media had a lot to say about SA, yet we've seemingly got a better safety record than them.
