I specified that I was not using madvpn, but regardless, I tried using ExitLag and encountered the same issue.

Hi, if exitlag did the same thing then there is something else at play. As you are then using their tunnels basically.
Is it only one game giving you that issue or others?

So the tracert seems normal? Is the madvpn.cisp.co.za host in lisbon/madrid reachable for you?
It appears that JHB -->LIS via Paratus is down at the moment, so madvpn.cisp.co.za would not be reachable, as we policy route it via Paratus only.
You can use madcptvpn.cisp.co.za in the meantime, but would be via Equiano(higher latency) vs paratus in comparrison.
Paratus doesn't have the best track record for uptime unfortunately, which is why we only use It for the VPN at the moment

Hi, When did these problems start?
You say stuff like twitch.tv, is it only twitch or other stuff?
Which router are you using? Are you obtaining DNS automatically?

In my experience, if turning on IPv6 breaks things its probably MTU related, since most devices will generally fallback to V4 if it cant open a connection on V6, but with MTU being wrong the connection opens but hangs on v6 without falling back.

This is also my suspicion which would then be the router doing a poor job of clamping.
Yeah there are a few things that don't fall back to ipv4 when v6 is broken. DSTV is one of them. If you have a v6 address and it's not routing DSTV streaming breaks until you completely switch off v6.
