For months and months now Telkom and Mweb have said my exchange is congested and have refused to do anything further. If it's congestion then where is the high ping from the first hop?! And why is my upload speed always fine? The severe packet loss is really killing everything.
 
Your first hop is your local router (192.168.1.1) which is why there is no packet loss. What you are experiencing is packet loss attributed to most likely a congested exchange. The reason why your upload speed (TX) is always fine/better is because the majority of traffic is subscribers downloading (RX) which means your TX traffic is competing less for resources. You cannot measure traffic loss across all the hops either as the packet loss is not between each hop but between your PC and each hop.

I have been in your position with worse stats (18-25% packetloss) and I eventually got a response from Telkom saying that they would be upgrading the links between the DSLAMs and the upstream exchange but it took a month or so for them to resolve it.
 
If it's congested then why are the pings not high? I have been complaining about this since October/November and they have done nothing.

Your poor down speeds are intriguing. Did Telkom measure the speeds from your location or did they just tell you that it was congested.
What is your attenuation and noise stats like?

Stats are pretty good and I never lose sync:

Connection Speed 4096 kbps 512 kbps
Line Attenuation 25.5 db 16.0 db
Noise Margin 23.9 db 14.0 db

Telkom didn't do a damn thing other than tell me it's congestion. I asked to be moved to another exchange and they said there is no other exchange to move me to. I asked to be moved to another port and they said there are no other ports available. I'm getting nowhere fast =/

Run the following command at your command prompt (I'm assuming you're running windows)...

ping -t -l 1024 openweb.co.za

Leave it running (CTRL-C to stop) for a while (a minute or so) and post the stats here.

What are your ADSL line sync stats? Also, what modem? Have you tried another modem?

I bought a brand new router to eliminate the possibility. It didn't help at all.
I have a Netgear DGN2200Mv2 and a TP-Link TD-W8950ND

Ok your stats are fine. So it's a problem on the exchange... the thing is your ping is low, so it's not congestion due to too many users, but rather a poor link of some sort... Try ask them to reset/reconfigure your port. If that doesn't work try asking them to switch you to a new port to see if that makes any difference. If not, it's probably the exchange backhaul that's faulty, meaning everyone using that exchange is probably experiencing the same problem

I've had my port reset more times than I can count. They say they can't switch me to a different port because they are apparently all full. I've been moaning for 4 months for them to fix it and I am no closer to a resolution now than on day 1 :(

Where do I go from here? Do I complain to ICASA or what can I do?

I think you have a congested exchange as has already been stated by Telkom. You need to get hold of Telkom and ask them when they plan on upgrading the exchange. If it is a congested exchange the test you did will get worse during peak hours - If you are in a residential area this should be 6-9pm. The larger the packet size the worse the packet loss should become. You can try 2048/4096.
 
Not congestion IMHO. That's a faulty network link somewhere in the exchange.

This seems to be a common misconception in this thread. There is no reason that you must have high latency when you experience congestion.

High latency is in fact a sign of a poorly configured router or network device. A correctly configured core network device should only add 5-10 ms of latency during even the worst congestion. Additionally - the user experience is far less degraded during congestion if it is properly done.
