Best American Prepaid sim for data and voice

From your experience, what is the best prepaid sim to get in the US of A.

I need it to come with at least 500Mb of data, some voice minutes, maybe some sms's thrown in for good measure. Need it for 2 weeks.

If you can shed some light on where to get this, if any paper work is required, and how much it will cost.

Depends on covrage, its not like here in SA where one SP covers the entire country.

cheapest Prepaid would be T-Mobile but again depends on the area's where you are.
 
Will be travelling around Florida.

Was in Michigan and even if the map said they had coverage it was few and far between. Purely an AT&T area :(

I am sure the SP's their have a standing contract between each other that they will not intrude on the others coverage. Well it felt like that. As for Pre Paid Documents they needed none. I had my Passport and they did not even ask for that.

When I was in Florida last year T-Mobile seemed to be the best for data, I know you can get an unlimited prepaid plan from them for $60.00, but I think that is data only, otherwise if your phone doesn't support CDMA (Verizon, Sprint & I think Virgin) you're gonna have to go with at&t

Got a Samsung Galaxy S3. So I can use t-mobile or at&t?
