In the past you automatically lost it unless you applied to retain it before you got a new citizenship. The DA recently got this law changed though. Not sure if it applies to Musk who left in the 80s.
DA ConCourt victory protects citizenship for South Africans
The Constitutional Court ruled today in favour of an application by the DA to secure the citizenship of South Africans with dual nationality.

It would have only affected people from 1995 onwards, the law wasn't retroactive so would not have applied to Musk as far as I understand.
