8ta modem settings

Good day to one and all , i recently purchased an 8ta sim card to use for internet with my Bandluxe C270 modem and iam failling to connect to the 8ta internet service . Each time i try to connect i am prompted to enter the following information :

PROFILE NAME
NUMBER
USERNAME
PASSWORD
APN.

If i enter the username and password i was given bu 8ta ,nothing happens .

PLEASE HELP .
