I've downloaded two versions of scanners (SCAN & QR Driod) on my Samsung SII.

I am unable to scan any QR code, my camera makes all the images blurry. Am I doing something wrong? Has anyone experienced this?

I had the same problem a while ago. Fixed it by cleaning the camera lens on my phone.
 
I cannot take any close up pictures, they always come out blurry.

Don't think it needs to be zoomed up to the max to work, back of a bit till you get focus. Also tap the screen to manually focus it.

Manually put the Focus Mode on Macro. I find that works MUCH better than Auto focus (specifically for close up photos).
Don't forget to put it back on Auto once you're done.

I've never had a problem with auto focus. Works perfectly. Also don't have a blur problem.

Either your lens is really dirty or there is something wrong with the camera. Could even be software related if you're into modding your phone.

I don't mod my phone. Other pictures come out perfect.

Tested with my SO's (also a Samsung hers SIII mine SII). Hers works 100%. Seems like my autofocus doesn't work whether on or off.
