Any mechanics able to identify engine issues by sound?

Like the others said its coming from worn lifters, rocker or cam lobes or a combination of them. 200K is a good amount of mileage though so it will likely need a rebuild at this point or your just going to be doing things one by one for a while.
 
This is easy. By the sound of it, it's gone wrong. You need to get it to sound right again.
Issue identified by sound.

Seriously though, that sounds like kicking a skeleton eating corn down stairs.
 
At 200 000km it's about time to sell it and get a lower mileage in my opinion - BUT if you did belts and timing, pumps, major service etc... it will easily take you to 300 000km after you sort the lifters and take a look at the condition of the engine.
 
If I need the lifters replaced, is it a major and expensive job?
total should be under 4k, Its not something I would recommend doing at home,

Rather find a mechanical shop willing to do it for you.
