Best ISP/Fibre for hosted voip

Hi All,

I'm in Durban, running a 16 user hosted voip network.
Hosted voip provider is Switchtel.
Isp is Cool Ideas
Fibre network is Openserve.

My problem is that a large number of calls are being lost.
I'm sure its the ISP/Fibre network.
What is the best ISP/Fibre combo to run a hosted voip network ?
