Burn an MDS file

Guy need some help please.
I am trying to burn an MDS data file (600mb) onto a disc. I know I can use Nero or Alcohol but what I need to know is the following: I need to put another file on this disc with the MDS file. The problem is that because this MDS file is like an ISO I do not know how to do the two to the same disc.

erm get something like poweriso and add the file to the iso, then burn it...
