...me with information on how to start up a small Wimax ISP, where to get equipment, licensing, etc? Would be greatly appreciated.

For starter if you serious
1: who offers wimax? (wikipedia mentions neotel, good place to start)
2: Equipment needed? (Google is your friend, http://www.netkrom.com/sol_wisp.html?gclid=CLe1z5_eqakCFcUMfAodMyO9Mw)
3: Feasibility study (is it really worth it and can I find the clients?)
4: Capital needed? (I’m pretty sure base stations are not cheap (if that’s what they called) can I afford this?)
5: Exit strategy (very important!!!, what happens if I F*** UP, how do I get out of this.....)
Off the bat, Google is your friend and you should have checked there first, secondly you should at least have a clue of the industry you want to break into.

Nooo! I'm not THAT stupid
ISACA or ICASA (always get them mixed up) are the guys where you will get spectrum from.
