If the exchange only supports 10Mbps there is nothing you can do about it, 23dB is how close you're to the exchange, you should easily get 10Mbps, if you're having random disconnects etc something at the exchange or along the line to your house isn't right.

Even the wiring at your house to the modem or even the modem itself.
