How do you formally declare any additional salary/income earned over and above your every day job?

I need to send through salary slips for a loan, but would also like to include my additional private income that i earn, Obviously I have no payslips and clients usually just deposit into my account.

You can set up a cash flow balance sheet in Excel and also supply your bank statements as supporting docs

Do you think a bank will accept a self drawn salary advice? The 3 months statements have been submitted also but they do not see the additional deposits into the account as it does not say "Salary".

I don't know. Did you declare it on your income tax? Maybe use that too.

EDIT: Remember that irregular odd payments don't provide the stability that a salary does. So it's up to them to decide if they wan't to use it in their calculations

Do you have contracts/invoices for your clients that would prove that it is genuinely earned income?

You can use your income tax returns

Equivalent to doing an EFT to your different accounts with the reference as Salary?
