Do you not have a laptop/computer..? This is simple to do on Adobe PDF products..

There are quite a few online PDF editors online not all of them are free, will have to search a bit to find the completely free ones.

Hellosign or docusign (both web based) should do the trick too

On my android i use Adobe fill and sign. Seems to work fine.

The built in pdf reader allows you to do that...
