“Structural shortcomings and defective designs” added to “unnecessary overtime” could be contributing factors to the September 2018 explosion at the Rheinmetall Denel Munition (RDM) factory in Western Cape, an ongoing inquiry heard.

The inquiry, conducted by the Department of Employment and Labour (DEL), resumed in Salt River last week with further evidence hearings to come on as yet unspecified dates. Information in this report is taken from a DEL posting on as government website. Reacting, RDM public relations officer Ruby Maree noted it was “biased” with “certain statements cherry-picked to put RDM in a negative light”.

The overtime claim follows earlier testimony indicating there was “a push” for production over the weekend before the explosion which killed eight employees. This was apparently to provide sufficient material for the N16 plant where CBI single base propellant is blended, a DEL statement said.

There was “negligence” at the time of the deadly explosion at Rheinmetall Denel Munition (RDM), which claimed the lives of eight workers three years ago.

It emerged with further witness testimony in the commission of inquiry into the 2018 explosion.

Final testimonies will be heard on Friday during the public inquiry by the Department of Employment and Labour to investigate events leading to the incident.
The inquiry is being held at the Salt River Community House.
Testifying virtually on Thursday, retired RDM employee Ernest Hodgson and former explosives manager, said the modification committee which he also served on submitted a review for the iris mucon valve which was in use at RDM, but the process was incomplete.
“There was a request in 2017 for a risk assessment to be done but the risk assessment has not been done…
“If a modification was installed and the modification had not been signed off, it cannot be used,” said Hodgson.
Representing the grieving family of the workers, advocate Winston Erasmus, probed Hodgson on what the action would amount to if a modification was fitted in the absence of a formally approved risk assessment process.
“Negligence” was Hodgson’s answer.

Any word from management? If it's a compliance issue, then why is management not being made accountable?
