Game keeps minimizing.

I have a problem, every time I play a game on my PC the game minimizes and I have to click on it on the start bar to continue. A few minutes in it happens again. It doesn't stop. It is really annoying.

I am using an Xbox controller for windows and the game is displayed on my TV and not my PC monitor.

Any idea what the problem could be?
 
Could also be fault reporting;

Update 2: I just opened Task Manager and found that the file named: "wermgr.exe" (Windows Problem Reporting) It just opens and closes over and over again. So, I pressed the start button and searched for "Choose how to report problems" and then I clicked it and then I clicked "Never check for solutions", Which fixed everything and now I can do what ever I like without This **** bugging me". Thanks for anyone who helped me here.

This happened with me when I use dual screen display. Ghost mouse goes to other screen. I right click to zoom in with my weapon, screen minimizes. I have no idea how to fix this apart from disabling dual screen when I play. Quite annoying.
 
Wanna hear something freaky/strange? I often play online (Steam) games with friends while on Teamspeak and many times our games would minimize at the exact same time. No idea what triggers it.
 
Had the same problem. Turned out it was a 3g dongle software that was trying to do something every 10mins. (maybe trying to auto update). I uninstalled it and the problem was gone :)
