If name doesn't matter, why add it? Don't include unnecessary information in your survey, especially name. It will only scare off potential respondents...

If you want decent survey results, add some more demographic information, like gender or marital status. You could then analyze your data based on age-groups, gender and marital status, i.e. single guys aged 20-25 play on average 10 hours of FPS games a month.
