My father is getting quite old, and we want to specify somebody as his medical proxy - I think that's the term? He has a living will already, but we need something to confirm that one of his children has the right to make medical decisions on his behalf. With his consent, of course!
I found this "advanced directive planning guide" which is very useful, but as far as I can see it doesn't specify how exactly to draw up a document that will specify who my father's medical proxy is. Unless I'm just not seeing it in all that text!
Is there a template for such a document - not for a living will, but for appointing a medical proxy?
