Is there a monthly fee for the "line", is this covered by the installation fee? Where does the "line payment" come in?

All depends on the provider. I know with Vodacom FTTH, instead of a 'Line fee', they tie you into a contract so you have to pay an initial installation fee (which they waive if you're an early adopter). Then you have a monthy fee of R899 and if you cancel there are cancellation fees. If at a later stage you want to reconnect, the fee is R899.

There is normally a one-time installation / connection fee for connecting your premises to the fibre.

The monthly charges are, in essence, the same as ADSL.The monthly ISP fee (for bandwidth) may (or may not) include the monthly "line" fee: if not, you will also have to pay the fibre supplier.
