Hydraulic clutch

Hey guys
First time posting. I have a problem with my 2008 Corsa utility hydraulic clutch, sometimes it doesn't clutch and the gears would not shift. We have changed everything from the sleeve, the release bearing in the gearbox to the pressure plate but it still gives the same problem and every time it messes up the sleeve. What could be the problem?
 
I am slightly confused about the "Sleeve" you are talking about, what is it? Could you be talking about the slave cylinder perhaps?
 
Okay so you have changed the master cylinder, the pressure plate and the bearing.

Did you properly bleed the system after? What about the slave cylinder?

While we were trying to bleed bleed the system, the master cylinder broke. Someone said the problem might be the might be the clutch bracket but the mechanic said the bracket is not faulty, and I'm not so familiar with opel
