Insurance vehicle release/clearance form

I'm after a generic template of the form you would sign when collecting your vehicle from a panel beater after repairs are completed. It's the form the repairer would submit to the insurer that confirms the client is happy and the insurer can pay the repairer.
Can anybody in the repairer or insurer industry share please?
