iPhone 4S no sound except ringtones works

HI All

I got the New iPhone 4S from Vodacom
No sounds work on my iphone except ringtones. Text tones, email, itunes, pandora, etc. does not play anything. Also, when I hit the volume buttons I get a picture of the speaker and it says "ringer" over it but there is no volume bar. I've cleaned out the headphone jack since that fixed a no-sound problem on previous iPhones but it didn't help.
I've tried turning bluetooth/wifi on and off because this seemed to help for some people but the only thing that helped with is that my phone now makes the noise when you plug it in to charge but still nothing else but ringtones.
Even tried restoring old IOS still nothing, then back to latest IOS.
Problem with mine is that it is intermittend, will just come back for a while then stop again.

Have you accidentally muted it? Depending on how you've got it set up check that. If you've got the mute/rotate switch on the side configured for rotate double click the home button and swipe all the way to the left where you see a set of playback and volume controls - to the right is a speaker mute button.

Nope mute not on.
Tested all of that. even restored software.
Still same issue. issue is it goes then comes back for a while then goes again.
At this moment not even working .
