Known error databse

What software do you use to set up an internal known error database? I've had a google, but mostly it comes down to using Excel or the best I've found thus far is Zendesk. Excel is too little and Zendesk is overkill.. Any other suggestions?

Maybe phpMyFAQ? Might work. Another option is Mediawiki, even though this might be a little too much.

You could consider a Github Jekyll site i.e. standard markdown.
Everyone submits as PRs, and the admins approve/commit.

If you need more functionality, just find the right template or make your own
