Got pretty much the same, I use both FNB and etfsa for my investments.
Note there is obvious overlap between some of them (eggs in one basket and all of that).
 
I think people are way too hung up on risk...largely because their focus is too short term.

The market ALWAYS recovers if you are in it for a 5+ years and therefore the only big risk would be investing in a single share/company.

So sticking to bundled options like ETF's or Unit Trusts are the safest way with decent gains I feel.

This is true. The T40 and MidCap is handled through FNB's ShareSaver. I've had it from the start, built up a nice some and now it is on autopilot with a small debit order every month.

The STXIND, STXFIN and DIVTRX is part of my TFSA (EasyEquities). The DBXWD and PTXTEN is part of a trading account (also EE).

I do have an RA and investment with Stanlib through a financial adviser and also keep some funds in a money market.

The most important thing is that I still have about 30 years left to work an no debt :)
