The picture contains the answer.

Front shocks are present, but the total absence of a rear shock absorber can and will wreck havoc on a man's prostate.

Also, perhaps the doctor got tired of performing prostate exams?

He had a femoral bypass in 2005, which was replaced in November 2022 (not bad considering they normally last about 5 years). The vascular surgeon told him to stop cycling after the bypass. In February 2023 he thought he was well enough and took the bike for a gentle 5km ride. That same night they had to do emergency surgery to replace the bypass again, so the surgeon suggested he sells the bike to stop any temptation of riding again.
