Is it worth it? Is the extra cost to extend the plan actually proportionally cheaper than maintaining the car out of pocket?

I stand corrected, just the warranty yes.Warranty maybe, no way a 3 y motorplan costs that. Especially not unlimited km's.
Just for info, Audi has recently dropped the price of its extensions. In may, my brother was quoted R22k per year for time extension as his car is low mileage.
Now with their pricing restructure in June 2019, he was quoted R27k for a 3 year extension, at Audi. This is comparative to 3rd party plans and much better to go direct if the pricing is similar.
Worth noting that they never offered 3 year extensions before. They quoted me a max of 2 years when I enquire last year.
I need to get a quotation for myself but have a few years left on factory plan.
Once you start adding mileage to the plan the pricing does increase.
Full maintenance plan. Their entire pricing structure has changed, effective June.I would like more details on this. Sounds intriguing. Was it full maintenance plan extension for 3 years? Or is it some sort of comprehensive warranty only? In the past you could only extend to 7 years or 200 000km maximum.

Depends. VW quoted R18k for extending Comprehensive plan by 2 years, for R18k. I said yes. as it was a gti, and already 5 yrs old with 95000kms. Audi on the other hand asked R30k for one year at the same point in its life, for an A1...
