Depends on the engine, some engines are freewheeling so if the cam belt snaps the car won't start but mechanical damage is highly unlikely whereas interference type the valves will catch on the piston.
1600cc CitiGolf engine from the late 1980's, early 1990's is one such engine.
My 1600cc Corolla engine will be damaged if the belt breaks, so I replace it every 100000kms. Cheaper to replace it than to replace the entire engine.
