Go to your nearest licensing department and take 2 copies his ID and 2 copies of proof of residence.
It takes about 2-3 weeks and they will phone you to collect the duplicate. You only pay at collection.
Not 100% sure, but i recall that the owner must go himself.

Where are you in the land, I went to Randburg and they give you the form there to fill, and also get a copy of ID. The lady there works for half day surprisingly so they close at 11.30am.
Besides that it's a pretty seamless exercise, only thing that annoyed me is after going thrall that trouble I found the original papers, now I have two certificates.

The title holder (owner) must be there in person.

Maybe your local office won't be closing at 11 like Randburg.

Unless something changed, Ive requested duplicate registration certs without being there in person.
