Unlocker won't remove the namespace but it will remove the files that run the namespace and hence remove the namespace.

What makes you think it's not a namespace? What opens when you double click the file?

windows picture and fax viewer, and i tried to open it with something else, no luck

not sure why it's there or what it's from...
maybe spyware?
Have you tried a spyware tool?
www.lavasoft.de
free.grisoft.com

Have you tried accessing the desktop with windows explorer:
C:\Documents and Settings\USERNAME\desktop

Delete it that way?

So you haven't bothered with adaware or the lavasoft app I take it?! I mean... it is spyware after all!

The fact that "Windows Picture and Fax Viewer" is being used to show the picture doesn't mean it's not a namespace but it does complicate the issue.

First thing I'd suggest doing is double click on the icon. Once the image has opened up in "Windows Picture and Fax Viewer", right-click on the image being shown and select "Properties", this should list the "Location" (i.e. file path) of the picture. Knowing the file path will allow you to delete the actual picture, the directory where the offending picture is located may also contain the files with the namespace extension DLL. If the directory looks suspect then it may be worth deleting the whole directory. If you run into problems doing this, "unlocker" will help :D

It may also be worth checking the "Add/Remove Programs" and seeing if there is anything suspect listed there.

Failing the above and after having performed a scan using "Ad-Aware" your next best option is to go through all the processes listed in the "Windows Task Manager" and end them according to how suspect they appear. You can Google most processes to determine what their actual use is.

Once you've completed the above exercise it's probably best to get your friend to read some safe-surfing guides :p
