Please help me to convert this PDF to an Excel file

Hey guys!
PLEASE (yes, I'm shouting) help me convert this PDF into an Excel file that looks exactly the same.

I have tried everything but nothing seems to go my way! Maybe one of you has a nice software that can do it, or maybe you are very skilled at replicating tables and forms.

May I ask the reason for Excel specifically? Will let you know if I'm successful :)

There are a couple of ways to do this....

1. Make the entire form excel. = Lotsa work
2. Make the form a jpg and then place it "behind" in excel.

The first one means the file would be smaller and more dynamic, so you could edit field names and so on.

The second would result in a bigger file, but would be alot easier. And you would only be able to fill in the form, not edit it.


EDIT:
Ok, So I had a look, and I can convert the form to a wmf, which means a much smaller file size, and no loss in quality from actual text because its a vector format.

OK so this is how you do it.

I can supply you the pages in a wmf format it you want them.

1.You then import them into excel
2. You format the sheet sothat there are cells behind every editable field.
3. Create borders around the editable blocks. (youre going to delete the other lines so that they don't get in the way when youre adding text.
4. right click on the picture and say "Edit picture". It will ask you if you want to convert it to whatever, just say yes.
5. Right click again and say "Ungroup all"
6. Delete the white background.
7. Go mad as you delete 40 million little lines around each editable feild.
8. when you get out of the mental hospital, lock all the other cells where all the 40 million pieces of text and lines are where you don't want to edit. Try not to go mad again.

And your done!

An easier way would to insert the wmf's into word, and then just type on top of the image.

I just need to be able to edit the blocks, the rest can be static. How would I go about converting it into a JPG?
 
I got it into word format if you want it? You'll need to make use of text boxes over the blocks to fill in your data though
 
I use CorelDraw to edit pdf's..

I can convert them to either jpg or Windows Meta File (wmf) which is a vector so its smaller and better quality than a bitmap (jpg) would be.

And because its wmf its windows native so it will work in any office app.

Gck2702 do you have photoshop?
Just open the pdf in photoshop and use the text tool to fill in the blocks. Let me know if you need help...
 
Ok I sent them.

They turned out a little bigger than I thought, but it should be ok.
