In the Catalyst Control centre, what 3D settings do you use?
For anti-aliasing, using the Edge detect filter can cause a huge FPS hit. Adaptive anti-aliasing set to "quality" can also drag FPS right down.
Besides that though, your PC should never display even the slightest lag with COD4.
Maybe you have a bad program running in the background? Check the task manager and see if you have anything showing significant CPU usage.
Maybe your PC is underclocked somehow. One possibility is weird Power settings, it's unlikely but if it is the case:
Go to Control Panel\All Control Panel Items\Power Options (you can type that as a file path in Win7 as a shortcut). Click "Change Plan settings" and restore defaults.
