Puk???

Hi,

I tried to connect a few mins ago and my dashboard says I have to insert my PUK?

I disconnected as per normal this morning and only tried to reconnect just now.

1) Anyone have any idea why / how this may have happened?

2) How do I get my PUK number? (I've misplaced my book with all my info)
