SARS E-Filing: Will she get a tax refund?

My housemate hasn't earned any taxed income the past year but has paid R18k to Discovery for her hospital plan (father supporting her).

On efiling it shows as 0 income.

Will she get the medical aid contribution refunded or only if she had a taxed income?

I.e. should she bother filing?

The medical aid deduction is a tax credit on taxes already paid for the tax year so no, she will not yet a refund as she did not pay any tax.

I would advise that she still complete the tax return and submit it. I've come across clients who were fined heavily for not submitting even though they technically we not required as per the SARS rules. I managed to get the administrative penalties waived in some cases and reduced in others.
